Community Partner & Resource Network
Stronger Connections. Stronger Communities.
Connect the organizations that already serve your community—so families reach the right support faster, without rebuilding relationships in a moment of need.
Independent partners
Shared community needs
Trusted connections
Why the Network Exists
Community Needs Don't Exist in Silos. Neither Should Our Solutions.
Mental health is connected to nearly every part of a person’s life. An individual seeking behavioral health support may also be navigating healthcare needs, education, employment, housing, food insecurity, family challenges, transportation, substance use, or other barriers.
No single organization can—or should—attempt to address all of those needs.
Instead, we can build stronger communities by creating an ecosystem of organizations that understand one another’s resources, recognize where their services intersect, and know where to turn when an individual or family needs support beyond what one organization can provide.
The purpose of the Community Partner & Resource Network is not to create one system.
It is to better connect the systems that already exist.

Different organizations. Different expertise. One connected community.
Every organization brings a different resource, perspective, or area of expertise. By strengthening connections across systems, we can create more effective pathways to services and respond more strategically to emerging community needs.
Impact
What Becomes Possible When We Work Together
Strengthen Referral Pathways
Help individuals and families connect with appropriate community resources more efficiently.
Increase Access
Reduce gaps between people who need support and organizations equipped to provide it.
Share Community Resources
Increase awareness of programs, services, events, trainings, and opportunities available throughout the community.
Address Service Gaps
Identify emerging needs and areas where additional community resources or programs may be necessary.
Develop Collaborative Solutions
Bring complementary organizations together when a community initiative would benefit from multiple areas of expertise.
Strengthen Funding Opportunities
Build relationships before grant opportunities arise so organizations can respond more strategically when funding aligns with shared community priorities.
Partnership Models
There Is More Than One Way to Partner
Referral & Resource Partner
Connect people with trusted resources.
Organizations serve as resources for one another by making appropriate referrals and sharing information about available programs and services. There is no expectation or requirement regarding the number of referrals exchanged.
Partnership Structure: Informal partnership acknowledgment. No MOU generally required.
Community Engagement Partner
Show up for the community together.
Organizations may support community events, educational activities, workshops, presentations, resource fairs, outreach initiatives, trainings, or other community-facing activities when opportunities align.
Partnership Structure: Generally informal. Event-specific agreements may be used when necessary.
Collaborative Service Partner
Bring complementary services together.
Organizations may coordinate programs or services for shared populations while maintaining completely independent operations. Each organization contributes its own expertise and remains responsible for its respective services.
Partnership Structure: MOU or written agreement generally recommended when defined roles, responsibilities, coordinated services, confidentiality requirements, or ongoing activities are involved.
Grant & Funding Partner
Pursue aligned opportunities together.
Organizations may collaborate when funding opportunities address shared populations, community needs, geographic areas, or organizational priorities. Participation may range from providing a letter of support to serving as an implementation or funded project partner.
Partnership Structure: Documentation depends upon involvement and may include a Letter of Support, Letter of Commitment, MOU, subcontract, or other project-specific agreement.
Strategic Community Partner
Build relationships that create future possibilities.
Organizations establish an ongoing relationship around shared community priorities and remain connected so opportunities for referrals, programs, community initiatives, workforce development, or funding can be explored as they arise.
Partnership Structure: May remain informal or may include a Partnership Agreement or MOU when ongoing responsibilities are established.
Important: Participation in the Community Partner & Resource Network does not create exclusivity, a joint business relationship, shared ownership, or an obligation to participate in any specific initiative. Organizations remain independent and determine when and how they participate in collaborative opportunities.
Build Relationships Early
Build the Network Before You Need the Network
Too often, organizations begin looking for partners only after a need emerges — a family needing a resource, a service gap, a school in crisis, a funding opportunity, or a new initiative requiring outside expertise.
By then, organizations are trying to build relationships while simultaneously responding to the opportunity.
We believe relationships should already exist. The Network creates space to learn about one another, understand available resources, and build trusted relationships before an immediate need arises.
Instead of starting with
“Who do we know?”
We can start with
“Who in our Network is positioned to help?”

We are not trying to become every resource a community needs.
We are building relationships with the organizations that already are.
